class=483211015-24102011></SPAN>which will be changing, the corresponding
request for change to academic and services fee form must be submitted. As
in the past, duplicate submission occurred occasionally when fee changes
were submitted in multiple spreadsheets. Therefore, we prefer each
institution or campus submit all fees change forms in one spreadsheet under
different tabs for each request form.</FONT></SPAN></DIV>

institutions propose fee changes as well as approval processes on

applicable columns before submitting request forms to

on Current Fee, Proposed Fee, and the Change</FONT></SPAN></DIV>

assessed by Per Credit Hour, Per Course, Per Semester, or
Annually</FONT></SPAN></DIV>

assessed to Lower Division, Upper Division, Graduate Level, or Professional
