I am not sure if I understand the question, but I don't see any problem
with borrowing survey questions wherever you can. Our campus copyright
expert (who is a strict constructionist) has informed me that its very
difficult to copyright protect survey questions.  I have always kept a big
file of survey forms categorized by type (alumni, new students, faculty,
etc.) just for this purpose.

COPLAC IR List Members:

Our institution is looking at best practices for tuition discounting.
Would you be willing to share the percentage of stated tuition that
enrolled full-time, first-time students pay?  I would be happy to share any
results that come in.

Nancy,

Public Colleges in NC are now allowed to discount tuition.  Students
receive scholarships but their resident or non-resident tuition and fees
are paid in full by somebody with non-state funds.
