URL: From counselors at lists.onenet.net Fri Dec 19 13:27:53 2014 From: counselors at lists.onenet.net (OSRHE EPAS PK-12 Counselor Discussion Board) Date: Fri, 19 Dec 2014 13:27:53 -0600 Subject: [Counselors] Outside agency Parent permission form In-Reply-To: References: Message-ID: Our school requires: 1) A yearly background check 2) Consent for School Based Services (signed by parent and the agency counselor) 3) Meeting with the building principal or counselor 4) Sign in at the front desk and sign out We have a meeting at the beginning of the year for all the agency counselors to come meet in principal of both lower and upper elementary as well as counselors. ?At that time we go over the requirements. ?Counselors are only allowed to see student one time per week during the elective hour only. ?The exception to be allowed to see student twice a week is if there is something going on (such as high risk/recent hospital discharge type situation). ?We have a compact agreement between agency and school which allows for communication. ?Some counselors do not like all of these rules but many understand as well. We also have the service provider give us their business card, copy of their license, a background check, and a copy of their insurance - all collected and approved by our special services office. Once that has all been done and the provider is allowed to see our student(s), the office let's us know and requests elective times from us (counselors). These are the only times they are allowed to see our students. We do not allow them to see students during core classes. Our providers have been very respectful of that with us. Some of our agencies have a "Consent for Release of Confidential Protected Health Information" form that parents and students can sign which allow them to coordinate services with the school. We have found this extremely helpful with several of our kiddos. I often let the outside counselor know of behaviors and/or issues that we are seeing here at school and ask if there is anything we can do to help support/reinforce any therapy goals. I try to be very respectful of their need for confidentiality and trust they will share with me what they feel is appropriate in order to best coordinate services for a student. I do not have electronic copies of either of these forms, otherwise I would share them. I will see if I can get one though. First the outside agency must be approved by our Board of Education. Agencies in the community may apply to be added to the list twice yearly. Each agency will be reviewed and must meet certain criteria before being presented to the Board members. Board approved agencies are then communicated to building administration. Representatives from the approved agencies must bring a letter of approval from the district office to share with the building principal before they may see the student. In addition the parent must sign permission for the counselor to meet with their child during the school day. Finally, and most importantly, the building principal may determine when and IF the counseling may take place on campus based on the instructional needs of the child. Participating agencies are required to sign a contract with the district indicating support of these policies and procedures.
