When you say students who struggle with a portfolio and students who can't do a portfolio do you mean "gap" students who do not qualify for the alternate assessment, but who are significantly academically lower than their peers? If so, I give the regular assessment (because that's the only choice we have) with any and all accommodations that is in the IEP that I can use for the assessment also. Likely the student will fail the EOI. When they do, I give it to them again during every testing session available until they make at least one point higher than their original score. I then give them a modified proficient score, add it to their IEP at the next meeting and it counts as a "pass" for that EOI. 

They can also complete a project. These are not required to be completed at school so the student is allowed to take them to get any help they need to get it completed. Your school will have a grading committee who grades the projects on site. We allow our students to ask questions of the grading committee if they need to.
