Laura,

 I am working on adding the personal financial literacy statement to
transcripts and this statement is on the front, "The Personal Financial
Literacy Passport is a cumulative record documenting that the student has
completed the requirements of Personal Financial Literacy." I take this as
the form needs to be in their cumulative file regardless of the class they
received their instruction. I have our teachers fill the form out so I can
put it in their cum file.  I hope this information helps.
