We also have forms we fill out that gives the parent the resources. Also, it lay out the problem the student is having because a very high percentage of deaths by suicide have sought medical attention within 6 mos prior to death. The problem is they see the dr but don't say what the problem is. Will be more than happy to share our forms!

I am concerned that a school may be setting itself up for liability (cost wise) by stating the parent needs to seek treatment and get a release.
Furthermore, as I'm sure you will, run the forms by the principal. They should be signed off on.
