[Counselors] Mental Health Eval Letter

[OSRHE EPAS PK-12 Counselor Discussion Board]

At the Retreat at Tatanka Ranch it was mentioned that there is a letter of
acknowledgement for parents to sign when their child has indicated they
want to self harm. Can someone send me either the link to that letter or a
copy of what your school uses.  We would like to have one on hand.
