[Counselors] documentation for students threatening self harm

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

Hello All!

We have a letter that we have parents sign after conferencing with them
when their child has threatened self harm.  The letter includes a list of
local Resources and states that treatment is the parents choice and
responsibility.

I am wondering what other schools use for documentation.

Do you file this in the students cumulative file?

Do you send this with other records if the student moves?
