[Counselors] FERPA

According to law, 18 year olds are independent; however, our school district has a board policy that says the student must sign in-house paperwork declaring independent status upon 18—otherwise, we still contact parents.

We have a student that has turned 18 and doesn't want her parent to receive her grades because she isn't living with her mother any longer...however she hasn't changed that status with the school.  Does anybody know of any official FERPA or HIPPA paper to sign?? And do all 18 year olds have to sign something?  Thanks for your help!
