[Counselors] Desk Monitoring Help

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

Hello All!   We have ALMOST made it!  LOL

I have been working on my documentation for desk monitoring.  I am having
difficulty with our school plan for Testing Status Application  vs Test
Irregularity Form..
Obviously we are following the proper procedures, however,  I am at a loss
of how to word that for the needs of the requested documentation.

Would anyone be willing to share an example of how they worded theirs???
