[Counselors] question about surveying students

Here are my thoughts

1- are you keeping up with checking your email and responding?

2- how are you doing emotionally?  feeling stressed?  disconnected?  How's your family doing?

3- does your family have everything they need in terms of food, services?   (are there local resources you as the counselor can connect them with if needed)

4- Do you have reliable internet access and a way to complete/submit your assignments?   Do you understand the instruction in the way it is being presented?

5- what can I do to help or support you right now?  (can you communicate with teachers on the student's behalf, or talk with their parents for them about the student's stress level, etc)

Our teachers have a group of students that they are supposed to call (beyond their classes) and check-in with every week.  I have been asked to create a survey/list of questions for the teachers to ask the students when they call every week.  Since several brains/counselors are better than one I thought I'd shoot this out here and see what amazing questions you guys have.  Thank you! I hope all of you are staying home and staying safe!
