[Counselors] Parent consent form

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

I plan to include in my enrollment packet a consent paper for parents to sign giving permission for ACT and PreACT scores to be on our transcripts. Instead of reinvent this, does anyone already have one made up they would be willing to share please?
