Does anyone have a school policy for suicide ideation and other self-harm that you can share?

This is the packet I use every time and then attach a list of local resources and places to receive help. I make it clear that they are signinga paper saying they will take their child to get help within 48-hours and if they do not bring back proof of seeing someone I will have to call DHS.
