[Oacrao-l] Legal Name Change Policy

Amanda is correct, the legal name change policy is uniform across all OSU campuses and Oklahoma A&M schools.  We require valid ID (driver's license, state issued ID, passport or military ID) and social security card reflecting the new name.  https://registrar.okstate.edu/site-files/documents/correction_or_change_of_name_form.pdf

We require a copy of their driver's license and social security card. I believe this is a University-wide policy in the OSU system.
