It is my opinion that the clarification provided is in error, but would like to solicit opinions. Situation: Student was selected for verification. Both parents are non filers and so stated on the verification worksheet which was submitted with the student signature and one parent signature. The guidance provided is that the verification worksheet is not sufficient in the case of non filers. That either two parent signatures is required (a signature for each non filer) or to secure a non filer statement from both parents (both parents signing) IN ADDITION to the verification worksheet and states this is required by federal regulation cited below. It has always been my opinion that a valid and complete verification worksheet is sufficient, with one parent signature and further have never been cited for this in a Program Review or audit. Have I been mistaken all these years? Thoughts? The clarification given is provided below. "The regulations do require a non-filer from each individual non-filer. I think you are mixing the VF requirement for one signature with the non-filer requirements. I am forwarding an email that Vicki's cousin wrote which will explain to you what is going on... On Monday she had a kidney biopsy and they took some bone marrow...as I am sending this I just received a call that they are taking her back down to remove fluid from her lungs again. This was extremely painful for Vicki so please pray that this time will go much smoother for her. Thank you all for your calls and support!!! Kristi Her internal medicine doctor had been by with a preliminary diagnosis around 11:00. Shortly after I arrived, the kidney doctor came in and said that the results of the kidney biopsy had come in and Vicki has a condition called TTP (Thrombotic Thrombocytopenic Purpura). This condition is very rare and depletes large numbers of platlets. Another condition, HUS (Hemolytic Uremic Syndrome) goes somewhat in hand with this. The treatment for this is to insert a catheter in collarbone area and being plasma exchange. We talked at length to the doctor concerning Vicki and how concerned we were and he said if hind sight were 20/20, he should have done this biopsy last week, but her levels did not present itself to be this condition. Vicki is what they refer to as an Atypical Presentation. Three different doctors used that exact term yesterday. She is NOT a text book case so therefore, they were puzzled by all the things going on. She did not present the classic 5 symptoms they were looking for with TTP. This is associated with ecoli as well. After visiting with this doctor, they immediately got her ready for this cath insert. We went down to the CT area and the Physicians Assistant who removed the fluid off the sack of her lung area several days ago was going to do the procedure. His name is Tom and he is a saint. The Radiologist that performed the kidney biopsy on Monday saw us in the holding area and wanted to know what was up. He and Tom worked closely together and he said, this is serious and I want to make sure that Vicki is taken care of and that we get this done immediately. They took her in and got the cath inserted. Both of these men are wonderful and have the greatest bedside manner and truly have Vicki's utmost interest at heart. When we arrived back upstairs, they indicated that Red Cross would be coming at 6:30pm to begin the Plasma Exchange. I have never seen the process of what this procedure. The Red Cross sends a person to do it. A huge machine was brought in the room and the lady came with her little red/white ice chest with FFP (Fresh Frozen Plasma). It is quite an ordeal to get it all going. They had decided to do a 1 gallon exchange, so therefore, 13 bags of plasma were brought in. Each bag has to be matched with paperwork and logged in. She finally got everything hooked up and began the process. This actually takes the blood out of Vicki and runs it thru a machine where the old plasma is separated from the blood thru a centrifuge and as it is processed to go back in, the new plasma goes back into Vicki. We were able to be in the room for the whole procedure and we got to see the old plasma that came off her. It was a greenish pond colored looking yuck. The Red Cross lady said it was pretty yucky and that each day it will improve as the old comes out and the new goes in. The doctors have ordered 7 treatments of plasma exchange. They do one per day. This can also be done as out patient if Vicki goes home and still requires more exchanges. In addition to the plasma exchange, she will be treated with steroids, Prednisone. She may have some more swelling but the doctor said it would actually help with her breathing. They have not made a decision to dialysis as yet, but the cath is in, so if they do decide to do it, she is ready to go. Her creatinne was 3.8 yesterday. It has been going up since last week. Her kidney output is still weak and they have stopped the Lasix. They may put the foli cath back in today. She was not to thrilled about that but the kidney doctor indicated he may want to do it again. A new doctor came on board yesterday that is a Hematologist/Oncologist. He came in the room yesterday around 6:00pm and talked to Bill and I extensively about Vicki. He is very concerned. Because he is new to the case, he wanted some info about how all this came about. He did tell us that this is very serious and the plasma exchange and steroids is the proper treatment for TTP, but he is still wanting to get the results of the Bone Marrow. Those results should be in today. The bone marrow culture will take a little longer to get the results. He did tell us she did not have leukemia but he is anxious to get the other tests results in. If you are interested in learning more about TTP, you can go the the Mayo Clinic web site: www.mayoclinic.com and click on Diseases and Conditions......scroll down to the word Thrombocytopenic and you can pull up all sorts of information about this condition. I do want to remind you that even though Vicki may start showing signs of improvements, the doctors have indicated that she is a very very sick person. Please realize that they are still asking for No Visitors as she always tries to talk even if you are just sitting there and she feels so badly if someone comes and they are asked to not stay or limit their visit. Vicki is always concerned that she is hurting someones feelings if she is not able to visit. I have assured her that she should not worry about that and that everyone certainly understands that SHE comes first and that we all want her well again. Only Vicki would be concerned about everyone else as she lies there so sick I hope that this e-mail is able to explain a little more and if you have any concerns, please feel free to call or e-mail me back. I do give Vicki the messages that many of you have via my e-mail. You should see her room, every doctor that walked in yesterday said, Oh my Gosh, she must be loved. TU students made a huge banner that is hanging on one wall and we have hung cards over practically every bare space we can find. She continues to have her flowers in the room. I try to exchange the water and clean out each time I am there. She has pics of the kids and she has probably received well over 100 emails. Again, that you all for your prayers and please continue to pray that this procedure will be successful and that all other results will come back good. Vicki is becoming very weary and her emotions are very sensitive. Steve is trying so hard to be such a trooper himself and he has been such a good husband and support to Vicki. I am asking now, that you please spend as much time today in prayer for Vicki. Attached is a copy of the agenda item which summarizes the data from the survey. Following are a few of the highlights from the data: * In 2005-06, students attending Oklahoma public and independent (private) colleges and universities received financial aid totaling $1.2 billion. Students at state system institutions received $940 million while students at the independent institutions received $261 million. Over half of the amount, some $656 million, was in the form of loans. * The annual rate of growth in the total amount of state system financial aid slowed in 2005-06 compared to previous years. The increase in 2005-06 was 5%, compared to annual increases of 16%, 11%, and 15% for the three previous years. * For the first time, the total amount of loans taken out by students within the state system in a single academic year topped a half-billion dollars at $507 million. * After four years of steady increases, the number of state system students receiving the federal Pell Grant actually declined by about 2,600 or 4%. The total dollar amount of Pell Grants awarded to state system students declined even more, by $10.6 million or 7%. This drop was due, at least in part, to a change in the federal formula that determines a student's eligibility for the Pell Grant. The change, which modified a formula factor for state taxes paid, eliminated Pell Grant eligibility for some students and reduced the amount of the grant for others. * The percentage of students receiving any form of financial aid in the state system increased to 60% from 57% in 2004-05. Included in the bill are changes to the current law related to the eligibility of undocumented students for resident tuition and state financial aid. The changes are quite complicated and our legal staff is still currently reviewing the bill to determine the specific implications of all the changes. The changes appear to impact eligibility for financial aid to a much greater degree than eligibility for resident tuition. If you have undocumented students attending your institution, you may want to have your legal staff (if you have them) review the bill closely. You can find the text of the bill at http://www.sos.state.ok.us/documents/Legislation/51st/2007/1R/HB/1804.pdf . Sections 11 and 13 relate to eligibility for resident tuition and financial aid. Section 8 may also have implications for high school students enrolling in Oklahoma's Promise-OHLAP. We'll try to provide more information on the bill in the future as the legal analysis progresses. SB 820 (Oklahoma's Promise - OHLAP) - This is the bill in which significant amendments to the Oklahoma's Promise-OHLAP program were approved in the House of Representatives (text showing the House amendments: http://webserver1.lsb.state.ok.us/2007-08bills/HB/sb820_hasb.rtf). Since then, the Senate has rejected the House amendments and the bill has now been assigned to a joint conference committee with both Senate and House members that will try to work out a final version of the bill. Below are a recent newspaper article and an editorial related to the bill. ======================= Going to college may get tougher for Oklahoma's Promise students By Tim Talley Associated Press May 7, 2007 OKLAHOMA CITY - When he was a junior at Oklahoma State University, Joe Dorman's interest in student government and a campus honor society left him with "C", "D" and "F" grades and sent his grade point average of more than 3.0 spiraling downward. Dorman, now a Democratic state representative from Rush Springs, says proposed changes to a tax-supported scholarship program that would require students to maintain a 2.5 GPA might mean that thousands of students like him would lose the opportunity to stay in school. "I was a good student, but I had one bad semester by over-involving myself with extracurricular activities," said Dorman, who attended OSU on a state scholarship plan that preceded the popular Oklahoma Higher Learning Access Program, also called Oklahoma's Promise. "Being vice president of the Sophomore Honor Society got me a flunking grade," he said. The GPA requirement is among several requirements the Oklahoma House tacked on to legislation that would create a permanent funding source for OHLAP scholarships, which will cost $48 million this year and about $60 million next year. About 17,800 students will go to a state college or university under the program next year. Under OHLAP, Oklahoma high school students can get a tax-supported scholarship if they take a rigorous college curriculum, maintain a 2.5 GPA, stay out of trouble and their families earn less than $50,000 a year. Amendments proposed by the Republican-controlled state House would require OHLAP recipients to maintain the 2.5 GPA in all but one "grace period" semester in college, meaning that OHLAP students would have to make at least half "B's" and half "C's" to keep their scholarship. Currently, there is no GPA requirement for OHLAP students. Other changes include allowing home-schooled children to qualify for scholarships if they score at least 22 on the ACT test, require OHLAP students to stay out of trouble during their college years, restrict scholarships to U.S. citizens and legal aliens and require that family income remain below $50,000 throughout the student's college career. The amendments have been rejected by the Senate and the bill is in a joint House-Senate conference committee to work out differences. Their author, state Rep. Randy Terrill, R-Moore, said they will bring accountability and an expectation of performance to OHLAP students. "It's about not just writing a blank check," Terrill said. "We expect him to get a good education so he can give something back." Oklahoma is below the national average in the number of college graduates as well as per capita personal income. "Statistics show that there is a clear link between the level of educational achievement and the amount one earns over a lifetime," Terrill said. But higher education officials said the GPA requirement will have a negative affect on the state's goal of increasing the number of Oklahomans with college degrees and could impact up to 30 percent of OHLAP students. "We would have some concerns that it might be keeping some out of the program who ultimately might be successful," said Higher Education Chancellor Glen Johnson. "It is important to provide opportunities for students to go to college. Anything that would not further our goal of more college graduates presents some concerns," Johnson said. The state Board of Regents for Higher Education said a review of 10,500 award recipients enrolled in state colleges and universities in 2005-06 showed that 37 percent of freshmen had a GPA of less than 2.5. In addition, 24 percent of sophomores had a GPA less than 2.5, 13 percent of juniors and 7 percent of seniors. The statistics reflect a trend in which the academic performance of many high school students drops when they enter the college environment. "If students don't have the skills to balance their classes and activities that freshman year, they're set up for failure," Dorman said. Statistics regarding Oklahoma college freshmen show their average college GPA at the end of their first semester of college was approximately a half-grade point lower than their high school GPA, regents said. "They do drop," Johnson said. "And then the longer that they are in college, the grade point average improves." Regents said a significant number of OHLAP students that make below a 2.5 GPA their first semester go on to graduate. Of the fall 2001 entering freshmen class, 44 percent of those with under a 2.5 first-semester GPA either completed degrees or were still enrolled five years later. "Our overarching goal is to produce more graduates," Johnson said. But a review of large scholarship programs in other states with similar college GPA requirements show that most students lose the scholarship. The Tennessee Education Lottery Scholarship requires a 2.75 college GPA after 24 credit hours and a 3.0 college GPA after 48 credit hours. A recent report shows that 50 percent of the scholarship recipients lost the scholarship after one year and that nearly two-thirds lost the scholarship by the end of the second year. In Georgia, the HOPE Scholarship requires a 3.0 college GPA. Studies indicate that about 50 percent of students lose the scholarship after their first 30 credit hours. At the end of 90 credit hours, the proportion of students retaining the scholarship may be less than one-third. Studies also found that Georgia's GPA requirements reduced the probability of full-time enrollment, increased the probability of course withdrawal and reduced the number of math and science courses completed by freshmen students. Terrill said he may propose postponing the effectiveness of the GPA requirement in Oklahoma until a student's sophomore year. Dorman said lawmakers should do all they can to assure the state has a well-educated work force. "We've got to make sure these kids have the opportunity to succeed," Dorman said. "Taking away the opportunity for students to succeed in college is not going to get us there." ======================= Editorial in the Daily Oklahoman Monday, May 7, 2007 Funding Promise: Scholarship Standards Shouldn't Change The Oklahoman Editorial Lawmakers are on the right track in trying to find a permanent funding source for the state's premier scholarship program. But they should be ashamed of themselves for connecting that change to proposals that would make it more difficult for students to keep scholarships. Different versions of the bill have proposed requiring scholarship recipients to have either a 2.5 or 2.75 grade point average in their college courses. The bill also would allow the scholarship to be withdrawn if students violate a school's code of conduct or commits a crime. The latter isn't a bad idea but would be difficult to enforce. Schools have different conduct codes, and there's a lot to be said for second chances should scholarship recipients mess up. Our greatest concern is with attempts to increase the GPA requirements and to keep otherwise qualified illegal immigrants from the scholarships. Despite comments that lawmakers want to make sure students are working hard, this smacks of an attempt to cut the program's cost. It would. But the price our state and students would pay would also be great. Students already in the program wouldn't be affected. But estimates from higher education officials show about 25 percent to 30 percent of future participants could lose their scholarships if the proposed standards are approved. It's not unusual for students to perform more poorly in their freshman year and then buckle down in following years. We have no reason to believe taxpayer money is being wasted on students whose GPA may fall below 2.5, and worry that those from low-income families will be most affected. Program statistics already show that OHLAP students have better retention rates and graduate at a rate higher than their peers. Changing the program standards at this point would be a mistake. If lawmakers are worried about the program's cost, then that discussion needs to happen separate from the process to establish a permanent funding source and include a look at how students would be impacted. ============================ Bryce Fair Assoc. I know I have not sent an update since Friday and here it is Wednesday so please forgive the lateness... Vicki had a pretty good weekend...her sister came up and stayed until Monday at noon and Vicki was so happy to see her!!! She had her last scheduled plasma exchange on Monday and they had also scheduled a dialysis session for that same day. On Monday afternoon they decided to cancel the dialysis as her numbers were looking better and they did not want to compromise the kidneys in any way. I can not tell you how much better her coloring looks to me after these exchanges...she is also getting VERY feisty and is still quite anxious. She is taking meds to help her with the anxiety but those 4 walls would be closing in on any of us for this length of time. She went out to the plaza last week for some much needed fresh air and even rolled into the gift shop on her way back to the room :-) We know how much she loves to shop so that definitely lifted her spirits that day :-) :-) :-) They are taking x-rays pretty often to check for fluid in her lungs...the one they took on the 7th showed pretty comparable to the one on the 4th. They are doing another today as her breathing is somewhat labored and they are not getting clear sounds when they listen to her. They said the fluid will come off slowly as she gets more protein in her body. I personally think I see a difference in her wrists as the hospital bracelets don't seem to look as tight as before. The kidney biopsy that was sent to Boston has come back and is confirming the H.U.S.T.T.P. The bone marrow biopsy that was sent to the Mayo in Rochester will not be back for a bit longer as it is a very precise test and it takes time to rule things out. They did another plasma exchange today and have scheduled 3 more after today. These will continue to be done until her blood counts stabilize. This is not uncommon at all to do more after the first initial round and can even be done as needed outpatient at the hospital after she is home. That is why they plan to leave the cath in for a while after she is home. Please continue to pray for the doctors as they treat her and pray for strength and courage for Vicki. We know she is a fighter and I witnessed first hand last week some of that tenacity she has which was so good to see as it had been way to long since I had seen it!!!! :-) While I was there this afternoon a hospital volunteer delivering "several" emails asked if she could talk to us for a minute. It was Vicki, the plasma lady and I in the room. She said she came in one day last week to make a delivery and Vicki was not there. She came in to leave the cards on the bed and said that she felt an overwhelming presence of God. She told Vicki that she has never seen so many cards in one room ever. Every wall is covered and the banner from TU takes up one entire wall by itself. She told Vicki that she has never had this feeling during all the years she has worked there. She told Vicki she can see what a lovely person she is just by all the love that is being sent to her in cards and emails. It was incredible to stand there and listen to her...she told Vicki that even though she does not know her she wanted Vicki to know that she is praying for her!!!! We all know how special Vicki is and even in sickness her sweet spirit is coming through to strangers!!!! Thank you for all the cards, emails, calls, but most of all the PRAYERS!!!!! Vicki had a good day Thursday. Her creatinne level has dropped to almost normal. They did another plasma exchange and plan to do another one today (Friday). I don't know if they will continue after today. On Wednesday they had said 3 more exchanges. Her Infectious Disease doctor is back on board as they have discovered some type of germ in her bone morrow that he says is rare that it is in the marrow. They plan to have her on IV antibiotics for 2 weeks to treat it. She is somewhat distressed and depressed as she is not wanting to stay in the hospital for another 2 weeks. It MAY be possible that the IV could be done at home but please know I am saying MAYBE. Please continue to lift her up. With this Sunday being Mother's Day, we know she would want nothing more than to be out of the hospital and that does not look like it will happen. Just pray for God's healing touch in her body. He knows every detail of what is going on when we do not have the answers. I know his time and our time are totally different, but please pray for divine answers as soon as possible. Vicki is being released from the hospital today!!!!! After 51 days of being sick and 43 days in the hospital you can only imagine how thrilled she is!!!! She will have another plasma exchange today before she is released. Has an appointment with the Hematologist/Oncologist on Wednesday and will have another plasma exchange on Thursday. From what we were told last week, she will have labs drawn at least once a week, then see the doctor and will do plasma exchanges as needed. The fluid is coming off slowly but is coming off...this could take up to 6 weeks to happen per the doctors. Please understand that she has a very long road ahead and will still need many thoughts and prayers!!! Steve's parents will continue to live there for the next few weeks. The Governor has 15 days (until June 9) to act on the bills. SB 820 - Oklahoma's Promise-OHLAP- The bill passed the Senate Thursday evening and the House just this morning. The bill makes several significant changes to the program including: * Providing a stable, dedicated funding source. * Requires a second income "screen" at $100,000 when students start college (becomes effective for students starting college in 2009-10 and thereafter). * Minimum college GPA: requires OKPromise college students to earn at least a 2.0 for courses taken during their sophomore year and a 2.5 for courses taken during the junior year and thereafter (becomes effective for students first receiving the scholarship in 2009-10 and thereafter) * College conduct requirement: Any OKPromise college student that is suspended for more than one semester from a college will permanently lose the scholarship. This provision becomes effective January 1, 2008. * Provides eligibility for homeschool students. * Prohibits students without lawful immigration status from participating in the program ("grandfathers" in students enrolled in the program by the end of 2006-07). The parents of a student live at a church camp and serve as camp director. They are required to live in a house on site because of the 24 hour on-call nature of their job. Their utilities are also paid by the Baptist assoc that owns the camp. Based on this information, we included $8400 in untaxed income and called it housing allowance on Worksheet B. The kicker is that I learned this morning after a much pleasant :-) call w/ the father that they actually have a primary residence nearby the camp that they pay normal living expenses for such as utilities and house payment. So, what do you think? Is it housing in this case? or is it a job requirement and not a benefit? I want to be as fair as I can be w/ the family (even though he has been less than nice about it) but I want to do it right. Thanks in advance for your input. I wanted to share with you the good news about Vicki...:-) She went to the doctor yesterday and her platelet count was.......118000 :-) :-) :-) this is awesome as she had been hovering around 50000. This is telling the doctor that the germ in her bone marrow is gone. Late last week they were contemplating doing another bone marrow biopsy but with yesterdays lab results he says that is not a thought any longer :-) Her hemoglobin is what it is for a "normal" person, as Vicki puts it :-) The fluid is coming off and from last week to yesterday she had dropped 35 pounds :-) :-) :-) There is still a slight issue with her lungs but yesterday was the best he had heard in weeks so that is good...that should get better as the fluid continues to come off. She was even able to wear her wedding ring yesterday!!!! Her feet and legs are still swollen but I can tell a difference. He is also reducing the steroid that is causing some puffiness and she hopes to be off it completely by the end of next week. He is going to keep a very close watch on her and she will have labs run about every two days. If they continue improving, then yesterday was hopefully her last plasma exchange!!!! :-) :-) :-) Please pray for strength and patience for Vicki. She is trying to do little things each day and that is helping but it will take a while to regain her strength. The Lord has answered our prayers and is continuing to touch and heal Vicki. She firmly believes that she has been given a second chance and she has a new lease on life!!! She doesn't remember much of anything about her time in the hospital; which is probably for the best! She is still having a hard time grasping how many of you have been praying for her and all the cards, emails and flowers you have sent. :-) She is now going through them one by one and reading them, which will take her quite a little while :-) :-) :-) I want to thank you for all that you have done...when I put out the request for prayer, emails and cards you all came through and went above and beyond what I ever could have hoped for. It is because of all your prayers and support that she is where she is today!!! Yesterday the doctor told her that she was just a mystery to him...she said, "NO, I AM A MIRACLE"!!!!!
