[Cobo] Updated CARES ACT/ARP Expense Report Form

In order to accommodate the reporting process for the continued use of CARES/ARP stimulus funding, please find the attached updated reporting form.

You will be required to update the columns highlighted in yellow that carries forward your cumulative allocations and expenditures from the previous report form for March 2020 - June 2021.

Please use this updated worksheet to report your expense activity through July 31, 2021, and then report each month thereafter, until the funding period is met. The following schedule has been developed as a calendar to submit the reports on a regular basis.
