[Counselors] In school placement after being dismissed from a facility.

[OSRHE EPAS PK-12 Counselor Discussion Board]

There is a re-entry form on the SDE website that can be used to hold a
meeting with a team to plan re-entry supports.  I have attached a link to
the form. You can find it by searching for the "Counselors' Toolkit" under
"Prevention Services."
