[Eoscstudents] COVID-19 Self Reporting Form for Students & Employees

Eastern Oklahoma State College now has an online COVID-19 Self Reporting
Form for students, faculty and staff at all teaching sites (Wilburton,
McAlester, Idabel, Antlers) who have been potentially exposed to or have
tested positive for COVID-19. Please complete the form immediately (within
24 hours) for any of the following conditions:

 

·         You have been potentially exposed to a case of COVID-19. Exposure
is defined as an individual who has been within six feet of a person with
COVID-19 for more than 15 minutes.



·         You have tested positive for COVID-19 or are awaiting test
results.



·         You have developed symptoms related to COVID-19 (Fever higher than
100.4° F; cough; shortness of breath or difficulty breathing; chills; muscle
aches; sore throat; lingering headache; diarrhea; loss of taste or smell).

 

Your information will remain confidential and will only be used to provide
support and resources, conduct anonymous contact training, and to ensure
compliance with Eastern’s protocols and policies for mitigating the risk of
COVID-19.
