[MainstreetManagers] Streetscape Business Survey

Hello Main Street Directors!

I hope everyone is doing well! We are working on getting our city to adopt
a streetscape but we are in the very early stages. The mayor wants us to do
a survey with the businesses to see what they would like to see (current
problems, things they would like to see, if they would support a
streetscape. etc.)

I know a few of you have already achieved a streetscape and some are in the
thick of it! If anyone has a sample survey that might help me, I would
greatly appreciate it if you could share!
