Hello everyone,

Please note that your December report will be due by, January 10, 2025.  Receive (5) Quality Assurance Points when reporting by the due date.

NOTE:  This only applies to all Designated programs.  For Associate programs, please feel free in reaching out to your state liaison to discuss and determine report completion/submission, as needed.  Network programs are exempt from completing/submitting.
