Rick, there are many days in which I wish I could make use of the guillotine :).  However, we actually don't note it as such on the transcript.  If we suspend the student then the notation simply reflects academic suspension.

Colleagues,
When a student has been convicted of academic dishonesty, how are you notating the transcript?  Do you give a student the opportunity to rehabilitate themselves & have the notation removed from the transcript?  Does your institution make use of the guillotine?
