[Oacrao-l] Legal Name Change Policy

Good afternoon to all!  The University of Oklahoma, College of Professional and Continuing Studies campus requires one to submit a copy of a fillable PDF form requesting the change, alongside a photo of a valid, active ID (driver's license is the most seen method).  We are looking into having an electronic form crafted soon.....
