[Oasfaa] Submission of Junior Concurrently Enrolled Students - Fall 2019 Semester

Please use the attached form to submit the concurrent enrollment waiver data for the Fall 2019 semester.   This will be used as the basis for the reimbursement for the fall 2019 semester only.  The spring 2020 Junior data will be added to your regular submission in the April 2020 timeframe for both senior concurrent waivers and National Guard Waivers.
