As was agreed during the Vocational Rehab session at the OASFAA
conference Wednesday, attached is the proposed Vocational Rehabilitation
Notice of Resources sheet. Once approved, the final version of this
sheet will be provided to the Department of Rehabilitation Services for
distribution to each of their counselors. It was agreed by Lynda
Lee Collins, DRS Division Administrator, that she would have her
counselors complete one on every Voc Rehab recipient each semester
payments are made. The sheets would be forwarded to you by mail or
preferably email. The attachment is a Microsoft Word
document. If you are unable to open it, please contact my
receptionist Carol Wichert at 580-774-3786 to have one faxed to
you.<br><br>
Our intent is to make this an electronic notification process soon, but
this is a first step allowing aid administrators in Oklahoma to receive
confirmation of Voc Rehab benefits. Please give me your comments on
this form <font color="#800000"><b>by 9:00 a.m. November
20th</b></font>.<br><br>

these forms to go to. I have been instructed to provide Ms. Collins
a full listing of addresses and a final version of this form by the end
