[Counselors] Outside agency Parent permission form

In my district we have provided policy and process for principals to follow before an outside counseling agency is allowed to enter the building.  First the outside agency must be approved by our Board of Education.  Agencies in the community may apply to be added to the list twice yearly.  Each agency will be reviewed and must meet certain criteria before being presented to the Board members.  Board approved agencies are then communicated to building administration.   Representatives from the approved agencies must bring a letter of approval from the district office to share with the building principal before they may see the student.  In addition the parent must sign permission for the counselor to meet with their child during the school day.  Finally, and most importantly, the building principal may determine when and IF the counseling may take place on campus based on the instructional needs of the child.  Participating agencies are required to sign a contract with the district indicating support of these policies and procedures.

We have several outside agencies coming to school to counsel our students during the school day.  One visits twice a week for more than a class period.  We try to schedule these visits during elective hours but sometimes this is spilling over to a core class.

My questions are:

1.  Do any of you have a parent permission form that you would share that states that the parent is aware of the amount of class time the student is missing. (or something like that)??

2. I feel if a student is counseled twice weekly at school, I should have some knowledge as to why.   Does anyone have a form to request this info from the agency??


Thank you for any thoughts or forms you would be willing to share.  Curious how others handle this situation.
