[Counselors] FW: SLO/SOO template

I wish we could have had a counselors’ meeting or webinar for an explanation on completing our SOO form.  We are a unique group and the samples I have seen do not “fit” for counselors.  I would appreciate any suggestions and any examples.

The form is in my OKTLE page under the blue SLO/SOO tab on the left.

The attachment that you show is a template but it has to be filled out in the OKTLE system and then signed by an administrator for it to be official.
