[Counselors] FW: SLO/SOO template

I talked to Jenyfer Glisson @ SDE and she said you do not have to have it in OKTLE.  It is easier for administrators to have it in the OKTLE for a report they will have to do in the future but not mandatory.  OKTLE is just a vender not part of the SDE.

The form is in my OKTLE page under the blue SLO/SOO tab on the left.

The attachment that you show is a template but it has to be filled out in the OKTLE system and then signed by an administrator for it to be official.
