[Counselors] Assessment surveys

[OSRHE EPAS PK-12 Counselor Discussion Board]

I am taking a class that requires a survey for parents, students,
administration and facility to better the counseling program.    Do any of
you do this and if so could I have an example?
Trying to get several so I can revamp them to meet my needs.  Thanks In
Advance.  Monica Grein Tonkawa Counselor (first year.....need lots of help).
