[Counselors] FW: Organization notes

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

Hi,
Attached is a pdf of the google form I created/use for contacts.  It dumps
the info into a google sheet (similar to excel spreadsheet) and also
creates some very nice graphs that can be used in reports and/or in
informing/advocating for your comprehensive school counseling program.  The
form took about 5 minutes to create using google forms.  Sooooooooo easy!
