We use our cumulative folders for more academic purposes and enrollment status (birth certificate, legal documents, etc.). We feel our forms are more on the personal social/emotional scale and kept separate. Our forms are shared during the transition time between our counseling team during our back to school meetings in August to know some kiddos they want want to be prepared to check on and be aware of any struggles they may have had the prior year.

With our high numbers in our larger district transitioning from year to year from elementary to middle and middle to high, we have found the folder/binders for them to look through the specific documents on self-harm/suicidal ideation versus checking each cumulative folder separately has been more effective. If we were to look through separate cumulative folders, we also feel we could potentially accidentally miss one if he gets stuck between the large amount of documents in the folder and you don’t notice it (if that makes sense). We just found more success with the separate files and then share those during transition time.

If the parent has signed the documentation that they were informed of their child's threat of self harm, why would that not be placed in the student's cumulative folder?  Wouldn't this information be helpful to the counselor at the new school should the student transfer?   Is this against Oklahoma SDE policy or law to place in folder?

Mid-Del has a framework we use that has a document probably similar to what you are talking about, except we have separated our form in to 2 documents: 1 is for self-harm and a 2nd form for suicidal ideation. These forms are in triplicate form so the parent receives one, the site counselor keeps one, and the other comes to me at the district-level to know what resources are needed or additional level of support depending on numbers. I keep the documents in a binder by site. This also helps with transition. We have been doing this now for 3 years and it has helped TONS in advocating on the what our sites need and the level of counseling support our students need. We tweak it each year depending on situations that arise and the “live and learn” moments.

I attached our packet to see what I mean. We typically do not keep them in the student’s cumulative folder, but instead our sites keep the documents in a folder or binder either I the counselor’s office or principal’s office. It does not automatically get sent through with records requests, but many times reach out to the site to make them aware. We have received some at sites with records that other districts send.

Hello All!

We have a letter that we have parents sign after conferencing with them when their child has threatened self harm.  The letter includes a list of local Resources and states that treatment is the parents choice and responsibility.

I am wondering what other schools use for documentation.

Do you file this in the students cumulative file?

Do you send this with other records if the student moves?
