[Counselors] Internship question

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

We borrowed from our alternative school & created an three page contract that states the student's; school's; employer's; and parent's responsibilities with the second page requiring signatures from all four.  The third page is a letter to the employer asking them to fill out details of the student's work experience.  This is returned to the counselor and kept in a file in my office.  We require that they bring documentation of hours (time card; work schedule; pay stub) at least once a month.  They are required to work at least 5 hours a week to earn 1 credit & 10 hours a week for 2 credits. I have attached the contract.

What does your school require for internships? For example, do students bring a copy of their paystub, sign an agreement with the school, require documentation from the employer?
