[Counselors] Opt Out form

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

Attached is an old copy we used prior to moving everything online.  We separated out the three groups, which a few of our recruiters gave us some guff about because they wanted it to be all or nothing, but many of our parents wanted to give access to just the directory or just the colleges/universities, so we continue to keep it separated out in three groups-it's just all done online now.  Sorry for the rough shape of the form...I could not find an electronic copy to send so had to scan an old one!

Does anyone have an example of your notification to parents/students that their name, address, phone number and email address will be released to the Armed Forces unless there is a written notice or 'opt out'? I have a paper (sent to me by my Army recruiter) and it states that parents must be notified. We currently don't have one in our enrollment packets.  Thanks!!
