[Counselors] Internship privacy form for medical internship

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

Good Morning counselors, I have students that would like to shadow or
intern in a medical or dental office but our local rural clinics so far
have expressed that they are not comfortable with it students because of
hippa/patient privacy.  Do any of you have a form or policy that someone
could share that might help address this concern?
