[Counselors] Survey needed

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

Does anyone have a "school climate" survey that they could share with me?  I am wanting to survey the teachers to see what they believe to be the main issues in  their classroom/school (i.e, absences, tardiness, disrespect, not doing work, etc.) and what they feel is/are major contributing factors,  (i.e, drugs, alcohol, phones, lack of consequences, etc).

I am wanting the  survey to be anonymous so teachers would be more willing to give input.
