[Counselors] example letter for retaining elementary student

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

We have a 2nd semester conference with parents to let them know that their
child is being considered for retention and have the parent sign it.  At
the end of the school year, if retention is being recommended, we have the
parent sign the attached retention recommendation form.
