[Counselors] example letter for retaining elementary student

[OSRHEStudent Prep PK-12 Counselor Discussion Board]

Here are some that we use at Soper. It is a very rare occasion that we
retain above 3rd grade. For Kg-3rd parents have to sign the APP (I think
it's called something else now) at each semester's parent conference
acknowledging that their student is behind in reading. That gives us an
extra parent signature in addition to the one below.
