[Oasfaa] Signature Forms for Excel and FTP Reports

Good Afternoon,

Schools participating in OTAG Excel spreadsheet or ftp reporting should be
reminded that your report is not complete until you have submitted your
signature form signed by an authorized representative.   We process the
reports only when they are complete with the appropriate signature.

We hope to be able to take advantage of electronic signatures in the future;
however, for now please remember to submit your signature forms when you
transmit your reports.
