[Oasfaa] Voc Rehab Final Form

I have completed the organization of our new Vocational Rehabilitation 
Notice of Resources form.  Changes recommended by many members of our 
association led to the final version.  I hope it comes to everyone's 
liking.  The first line was lowered to allow the Voc. Rehab. counselors the 
option of typing in the institution's contact information for use in a 
window envelop if mailed.

It is my sincere hope that further conversations with Lynda Collins, DRS 
Division Administrator, will lead to electronic submissions in batch once 
the ease of that process can be demonstrated to her.  Until such time, 
perhaps the attached form will give us all some well needed information.
