filled in and executed by each institution and entity and received back

our office by Nov. 21.  This is in regard to the questionnaire that each
institution and entity completed and provided to you for our review.  It
allows us to rely upon that information and any subsequent information
they
gave in order to render our bond counsel opinion.  If you have any
