[Nclb] REMINDER: 2003 NCLB Participant Information/2nd Quarter Payment

DON'T FORGET!   Please provide information on accepted 2003 participants using the attached form (Excel).   Please submit this form by E-MAIL to Marvanna Millican as soon as you have accepted all participants (20 minimum).    The second 1/4 of your grant monies will be forwarded to your institution once the information requested is returned on the attached form.  

> As you will see on the form, information to be provided to the State Regent is the following:

> *	Grade Level Taught
> *	Subject Taught
*	Ethnicity
> *	School Name
> *	School District
> *	Free/Reduced Lunch Percentage of School (This information can be found at http://www.ed-stats.state.ok.us/.  Go to the district report for the school.  The information is under the first heading "Socioeconomic Data."  It is the last item of that section. This is for public schools only.)

Upon completion of the NCLB workshop, please verify participants.  At that time, send another list BY E-MAIL of ACTUAL TEACHER PARTICIPANTS on the same form.
