We also ask, but it is optional.  Something else to consider...when it
is left blank, it is difficult to have solid statistics when you are
applying for grants. Typically underserved populations are hesitant to
self-identify, leaving us in a catch 22 situation.

for biographical information.

We ask the question by saying "The following information is voluntary
and requested for reporting purposes only in accordance with the 1964
Civil Rights Act as amended. Mark all that apply."  This gives the
option of answering or not.

There has been some recent concern on our campus over the collection of
ethnicity data on our application for admission even though it is for
information and reporting purposes only.  My question is how many of you
ask the ethnicity questions on your application for admission?
