Oklahoma State’s policy on diploma names is similar to what Julie describes below. I’ve attached our Diploma Name Preference form for more details.

Shelley:

At Northeastern, we use the name in the system with very few exceptions.  However, we do allow students to substitute their maiden name for their middle name without documentation.  Our policy is not to print nicknames such as Abby or Shelley.
