At the University of Oklahoma we have a several options depending on the academic misconduct.  First, is censure, which nothing is on the transcript.  Second, is  limited notation suspension.  It is on the transcript for four years or until the student graduates, whichever comes first, and then the notation is removed.  Third, permanent notation suspension.  It is on the transcript permanently.  Fourth is expulsion.  It is on the transcript permanently.

Colleagues,
When a student has been convicted of academic dishonesty, how are you notating the transcript?  Do you give a student the opportunity to rehabilitate themselves & have the notation removed from the transcript?  Does your institution make use of the guillotine?
