[Oacrao-l] Transfer Clearance Form for Conduct

Happy Monday to all of you out there in OACRAO-Land!
I hope this first day of the school week finds you rested and energized and ready for a great week ahead. I have a question that I'm hoping you all will answer for me.
Does any of your colleges/universities require any type of a Conduct Clearance Form from transfer students who are transferring to your college/university? Specifically, I'm talking about a non-academic report of the student's conduct at their previous school, signed by the Dean of Student's Office, disclosing whether or not the student was suspended, expelled or any other non-academic violation. The form may include the reason for suspension and possibly a question asking if they were eligible to return to your institution. Of course this does require that the student sign off on the form giving permission to disclose this information.
We've noticed that some out-of-state schools are requiring this form be filled out before a student can be admitted to their school, so we're just wanting to know if any of our state schools are requiring anything similar as well.
