The following are forms that I have used in the past.  Students were required to attend the Career Fair to be eligible to job shadow.  I hope the forms and information is helpful.

"Job Shadowing" PARENTAL PERMISSION Form

Dear Parents or Guardian,

     Hilldale Middle School is proud of our students who have participated in our Career Exploration programs.  This year eligible students will have the opportunity to perform a "job shadow" of a person in our community. The job shadow student will be released from school for the entire day on Wednesday, February 2, 2010 in order to perform this activity. The person being shadowed should be able to have the student follow him/her for the entire day (8:00-3:00). During this time the student will gain knowledge of the job and have a good understanding of the roles and responsibilities of the person being shadowed. The student will have a questionnaire to complete and will have to prepare a written report to be presented to Ms. Fenton/Ms. Dover and to their English class on their return.
     The parents/guardians should be aware that they are responsible for the student's transportation to the job shadow, and will accept full responsibility for their safety. Students should not "shadow" dangerous occupations that may be harmful or place them in hazardous areas. Students should refrain from shadowing occupations in the "fast food" areas or occupations located within the home.  We hope the students will gain knowledge and be able to view the roles and responsibilities associated with various occupations. Hello! I am hoping to help implement a Job Shadowing program at our school. Do any of you have a program currently in place? If so, would you be willing to share information about it and documents used (i.e. permission forms, etc.). I don't want to "reinvent the wheel" if someone has a good program in place :)
