[Counselors] Concurrent permission slips for counselor's to check grades

Our Local Colleges have a form that parents and students must sign with
them, granting us access.  If students don't sign it, we remind parents
that it is their responsibility to take care of all things related to
concurrent classes, including insuring that students are passing and must,
to remain eligible for sports after grades at semester.  Usually
parents/students decide at that time to grant us access.  If they don't, we
just enforce the rules. Of course, we are still a resource for our
concurrent students, and ensure that they appear to be doing all
assignments.
