[Cobo] Capital Bond Draw Instructions and Forms

Business Officers,

 

Please find the following documents attached to this email:

 

1.  Instructions WORD document

2.  Draw Cover Page  BBC-1a 

3.  AIA G702 Form pdf

4.  Affidavit forms pdf.

 

These documents should be used to submit cash draws on your bond
projects on the 10th and 25th of each month.  You may submit by email
pdf or by fax to my attention to 405-225-9230.  ALL invoices must have a
signature directly on them in addition to the appropriate affidavit and
the cover page.

 

Should the 10th or 25th fall on a weekend, please send your requests the
preceding Friday.

We were to discuss this process in detail this morning with the OCIA
personnel, however, because the 10th of the month is close at hand, I'm
sending to you through this method.
