[Cobo] FY09 Tuition and Mandatory Fee Forms

Attached are the forms for use in submitting institutional requests for
FY09 tuition and mandatory fees for undergraduate, graduate, and
professional programs.  
 
The forms this year have changed primarily in two aspects:  1) separate
forms are provided for each tier for completing Section I (Tuition) --
A) Differentiated Rate and B) Single Rate -- for undergraduate,
graduate, and professional programs;  and 2) the guaranteed tuition rate
columns have been added for the research and regional institutions.  (HB
3397 removed community colleges from the guaranteed tuition rate
requirement.)
 
Please complete the appropriate form(s) for your institution's tier and
programs.   Also, complete the narrative form documenting the impact on
students' ability to pay, enrollment, availability of financial aid,
cost-effective measures implemented, and communication to
students/constituents.  For each mandatory fee which will be changing,
the corresponding Request for Changes to Mandatory Fee form (also
attached) must also be submitted.  
 
The deadline for submitting the request forms is 5:00 p.m. on Friday,
June 13, 2008.  A tuition review hearing is tentatively scheduled to be
held on Wednesday, June 25, 2008.  Additional information will be sent
to presidents regarding the tuition review hearing schedule.
